Lindsay Surname DNA Project************************* Consent & Liability ReleaseIn the handling of your personal DNA profile results, it is the intent of the Lindsay Surname DNA Project Coordinator to ensure to the best of his ability the strict confidentiality of the specific identity of the participant in conjunction with the participant's personal Y-Chromosome DNA results. However, the individual DNA data for each participant will be posted on the Lindsay International web site, and possibly other future literary expressions (books, etc.) in the manner prescribed below. Unfortunately we live in a time when an individual, who chooses in good faith to expend the significant personal resources necessary to coordinate such a project, also potentially exposes himself to unwarranted liability. Thus, based upon advise and counsel, I have included the following waiver of liability form that will be required to be executed by all Lindsay Surname DNA Project participants before the Project Coordinator will integrate your DNA Profile data into the Lindsay Surname Haplotype Database and post it at the public Lindsay International web site. The form is shown below for your review and approval. ********************************* Start Form Lindsay Surname DNA Project Consent & Liability Release Form -------------------------- This Release and Waiver is executed in the interest of permitting the Lindsay Surname DNA Project to go forward in the genetic genealogical testing of all participants and of permitting Ronald G. Lindsay to serve the Lindsay Surname DNA Project as Project Coordinator. It is the intent of the Lindsay Surname DNA Project Coordinator to ensure to the best of his ability the strict confidentiality of the specific identity of the participant in conjunction with the participant's personal Y-Chromosome DNA profile. Therefore I, ______________________________________________________ [Print Full Name], give permission to Ronald G. Lindsay, Lindsay Surname DNA Project Coordinator, to receive my DNA profile results and affiliated genealogical information. It is also my understanding that the composite individual data from all participants in the Lindsay Surname DNA Project will be posted on the Lindsay International web site, as a copyrighted document, and possibly will also appear in other future literary publications, possibly using some or all of the information specified in items 1 thru 7 below. The information in items 5 thru 7 below is utilized to help identify the Most Recent Common Ancestor (MRCA) shared by two or more individual participants.
I furthermore assume responsibility for any risk associated with my participation in the Lindsay Surname DNA Project. I also agree to indemnify and hold Ronald G. Lindsay (Project Coordinator) harmless from all claims, judgments and expense incurred in connection with any claims brought as a result of my participation in the Lindsay Surname DNA Project and which are released by the terms of this agreement.
